Being hungry without food, and/or water is about the worst thing that could happen to someone
The only thing I can think is if you were hungry and thirsty and human trafficked for sex
That's about as bad as it gets, but then there's torture murder porn, that happens every day
So if you're not starving, thirsty, forced to have sex, or being murdered tortured it could be worse
